with performance expectations. Billing shall occur within three (3) days of
receipt of a completed demographics face sheet, copy of insurance card (if
available) and the dictation. The chart will be coded with the proper ICD/CPT
codes in accordance with the services rendered to the patient, as presented
in the dictated notes of the medical record.
